| 货物运输保险理赔所需的单据3 |
海运集装箱整箱运输，如货物发生短少，通常需提供以下资料：
1.正本保单一联；original policy of insurance
2.正本提单一联（如是电放，须提供清晰的复印件）；original bill of lading (If B/L
surrendered/telex-released, please give us a clear copy of B/L with the stamp of
surrendered/telex-released)
3.正本发票和正本装箱单Original commercial invoice & packing list with shipper’s stamp and
signature
4.买卖合同复印件（需在复印件上盖章，如合同是多页，请加盖骑缝章，并注明“与原件相符”）；copy
of sales contract with seller’s original signature and stamp on the copy and with the
statement “match with the original one”
出口报关单复印件；copy of export customs declaration
5.装箱照片或监装报告请一并提供；if any loading report before shipment please also submit
6.装运港场站收据；dock receipt at loading port；或装运港的进出堆场的设备交接单EIR(向负责运
送至堆场的司机要) EIR(equipment interchange receipt) from and back to CY/port at the
loading port
7.卸货港的EIR(向去卸货港拉货的司机要) delivery receipt or EIR(equipment interchange receipt)
from CY/port, which could be got from the driver who picked up the container from
CY/port
8.收货人收货时与卡车司机之间的货物交接单；delivery receipt between the consignee and the
inland driver
9.收货人说明集装箱到达收货人处时外表及铅封状况（由收货人签字、盖章）consignee’s statement
about the condition of the container when arrived(with consignee’s signature or stamp
