13.进口国检验检疫证书原件；original phytosanitary certificate/ Animal and quarantine
certificates issued by importing country government
14.进口报关单；importing customs declaration form
15.收货人收货时与卡车司机之间的货物交接单；delivery receipt between the consignee and the
inland driver
16.收货人说明集装箱到达收货人处时外表及铅封状况（由收货人签字、盖章）consignee’s
statement about the condition of the container when arrived(with consignee’s signature or
stamp)
17.开箱时集装箱照片、货损照片；color photos of the damaged cargo and the container when
opened the container
18.收货人与承运人（和其他责任方）关于损失索赔的函件、传真等；correspondence exchange
with the Carriers and other Parties regarding their liability for the damage
19.收货人索赔函和索赔清单（须列名索赔事项和金额）consignee’s claim letter to Ping An which
clarify the damage situation and the claim amount
20.如发生残值处理或维修，请提供相应的发票；if salvage action or recondition, please submit
the relevant invoice
快递公司寄送的货物如发生损坏或短少，通常需提供下列资料：
正本保单一联；original policy of insurance
正本快递运单一联；original waybill
正本发票和正本装箱单；Original commercial invoice & packing list with shipper’s stamp and
signature
买卖合同复印件（需在复印件上盖章，如合同是多页，请加盖骑缝章，并注明“与原件相符”）；copy
